License Reinstatement DUI/DWI

This assessment is for license reinstatement from a DUI/DWI charge and for those who are now trying to get their license returned along with driving privileges. We use the assessment tools that the U.S. Department of Transportation, National Highway Traffic and Safety Administration deem the best for accurately assessing client’s behavior and identify client risk. The assessment will gather information, identify driver risks and problems and helps to formulate specific intervention and treatment strategies if needed.

I know the results of the license reinstatement are very important to you and I take this process very seriously. I work very hard to make sure this process is fair, accurate and non-judgmental.

I am available for license reinstatement evaluations 7 days a week and provide professional evaluations in an expedited and timely manner. You can order regular service for your license reinstatement evaluation as well as expedited rushed service license reinstatement evaluation if you waited until the last minute.
The license reinstatement assessment will determine if there is a substance use disorder and whether or not education or treatment is recommended to minimize risks in the future. Not everyone who takes an evaluation needs treatment.

How it Works

  • Order the license reinstatement assessment evaluation.
  • The assessment forms are emailed, faxed or mailed to you.
  • Full instructions come with the license reinstatement evaluation.
  • I am available for you through the license reinstatement assessment process.
  • You fill out the forms and return them to me either by email or fax.
  • Once I receive the forms, I will contact you for questions and clarifications.
  • The completed license reinstatement evaluation is emailed or faxed back to you.
  • Because this is a confidential record, it will only be sent to you unless you sign a release form.

Important to note: This is not an online license reinstatement evaluation. Sometimes online services may be rejected because people may think that the evaluation is administered online and therefore does not have credibility. This is not the case with Center of Solutions, LLC.

We have a very high acceptance rate. I have been online providing these services since 2002 and have gained trust in the community and in my online presence. While my services are advertised online, this is the same evaluation you would receive and complete if you did an in-person evaluation. So if you are asked if this is an online evaluation, the answer would be no. You are completing the DUI/DWI evaluation with Dr. Kathy S. Garber, a licensed therapist with years of experience in the field of addiction and alcohol and drug evaluations.

Whether you are going through the court/legal system, employer or just addressing alcohol or drug concerns of your own, a DUI/DWI evaluation is a great starting point to help you decide your next steps, set new goals and determine your future.
